Vitamin B12 products are available as tablets, capsules, sublingual drops, oral sprays, nasal sprays, and injections. Marketing often suggests that sprays bypass digestion, injections are always superior, or methylcobalamin absorbs better than cyanocobalamin.

Vitamin B12 Injections

Vitamin B12 Tablets

Oral tablets use two absorption pathways. A limited amount is absorbed through the normal intrinsic-factor pathway, while a small percentage of a large dose crosses the intestine through passive diffusion.

This explains why a tablet containing 1,000 mcg can still deliver a useful amount even when normal absorption is reduced. NICE recommends at least 1 mg daily when oral replacement is used for confirmed or suspected malabsorption.2
